IF TEAMS FILL OUT A TEAM APPLICATION
FORM AND LEAVE A $400.00 DEPOSIT ON OR BEFORE: Sept.
15, 2010.
WENTWORTH ARENAS WILL ENSURE YOUR SPOT FOR THE UPCOMING SESSION. AFTER THIS
DATE, REGISTRATION WILL BE OPEN ON A FIRST COME FIRST SERVE BASIS. WE ARE
LOOKING FORWARD TO HAVING YOUR TEAM RETURN SO DON’T MISS OUT.

(Green Forms - made available at the main office of Wentworth Arenas)
also, make sure all players are listed on your teams
Roster.
(Rosters and O.S.A. forms are due the first game)
All players MUST be Registered.
***Teams have until their 6th game of the season to

any NEW players only, --the others should have been
